Where is the Iliopsoas Muscle Located?
The iliopsoas isn’t just one muscle, but rather a combination of two muscles: the psoas major and the iliacus. These two muscles merge together as they approach the hip joint and function as a single unit.
The psoas major originates from the lumbar vertebrae (T12-L5), meaning it starts at the last thoracic vertebra and runs down through all five lumbar vertebrae in the lower back. It also attaches to the intervertebral discs between these vertebrae. From there, the psoas major descends downwards and slightly laterally (outwards) through the abdomen.
The iliacus muscle, on the other hand, is located within the iliac fossa, which is the concave surface of the ilium (the largest bone in the pelvis, forming the upper part of the hip bone). Think of it as being “inside” your hip bone. It covers a significant portion of the inner surface of the ilium.
As the psoas major travels down from the lumbar spine and the iliacus arises from the iliac fossa, they converge. They pass under the inguinal ligament (a band of connective tissue running from the anterior superior iliac spine to the pubic tubercle). Finally, they both insert onto the lesser trochanter, a bony prominence located on the inner (medial) surface of the femur (thigh bone), just below the hip joint.
Therefore, the iliopsoas muscle essentially spans from the lower spine and inner hip to the upper thigh. Because of its attachments, it has a direct influence on both hip flexion (bringing the knee towards the chest) and stabilization of the spine.
