Understanding Muscle Invasive Bladder Cancer
Bladder cancer occurs when cells in the bladder, the organ that stores urine, begin to grow uncontrollably. When this cancer spreads beyond the inner lining of the bladder and invades the muscle layer, it is classified as muscle invasive bladder cancer (MIBC). This stage is more serious than non-muscle invasive bladder cancer because it has a higher risk of spreading to other parts of the body.
Muscle invasive bladder cancer requires aggressive treatment. The most common initial treatment is radical cystectomy, which involves surgically removing the entire bladder, nearby lymph nodes, and in men, often the prostate and seminal vesicles. In women, the uterus, ovaries, and part of the vagina may also be removed. Following a radical cystectomy, a new way to store and eliminate urine is necessary. Options include creating a neobladder (a new bladder made from a segment of the intestine), an ileal conduit (urine is diverted to a bag outside the body), or a continent cutaneous reservoir (a pouch created inside the body that needs to be emptied with a catheter several times a day).
Another treatment option is bladder-preserving therapy, which usually involves a combination of transurethral resection of bladder tumor (TURBT), chemotherapy, and radiation therapy. TURBT removes as much of the tumor as possible, while chemotherapy and radiation help to kill any remaining cancer cells. This approach is generally considered for patients who are not good candidates for surgery or who strongly prefer to keep their bladder. However, careful patient selection and close monitoring are crucial, as the cancer may recur.
Treatment decisions are complex and depend on several factors, including the stage and grade of the cancer, the patient’s overall health, and their preferences. A multidisciplinary team, including urologists, oncologists, and radiation oncologists, is usually involved in developing the best treatment plan for each individual. Ongoing research continues to explore new and improved treatments for MIBC, including immunotherapies and targeted therapies, which aim to specifically target and destroy cancer cells while minimizing damage to healthy tissues.
Frequently Asked Questions (FAQ)
- What are the symptoms of muscle invasive bladder cancer? Common symptoms include blood in the urine (hematuria), frequent urination, painful urination, and feeling the need to urinate even when the bladder is empty.
- What are the risk factors for muscle invasive bladder cancer? Smoking is the biggest risk factor. Other risk factors include exposure to certain chemicals, chronic bladder infections, and a family history of bladder cancer.
- What is the prognosis for muscle invasive bladder cancer? The prognosis depends on various factors, including the stage of the cancer, the patient’s overall health, and the response to treatment. Early detection and aggressive treatment can improve the chances of survival.
- Can muscle invasive bladder cancer be cured? While there is no guarantee of a cure, treatment can often lead to long-term remission. Regular follow-up appointments are essential to monitor for recurrence.
